Supporting Adults with Long-Term Condition

About this programme

This module is aimed at health and social care practitioners, and those with responsibility for care and management of adults with long term conditions. The number of people living with a long term condition (LTC) is on the increase.

This presents an enormous challenge to health and social care systems. Many people have more than one LTC that adds to the complexity of care required. The Government’s commitment to improve well-being for people living with LTCs has marked a radical change in approaches to care. New ways of working are emerging that promote a person-centred, collaborative approach, putting people in the driving seat in determining their own care needs.

This contemporary module aims to facilitate the development of skills and knowledge, approaches and behaviour needed to support the multifarious needs of people living with chronic illness. It moves away from the traditional disease focused approach, to one that centres on the cross-cutting commonalities experienced by individuals living with long term conditions.

A blended learning approach has been chosen for delivery of the module; therefore, there will be a combination of attending university for half a day some weeks and engaging in online activities in other weeks.

This course is delivered on Wednesdays on campus and online.
