This presents an enormous challenge to health and social care systems. Many people have more than one LTC which adds to the complexity of care required. New ways of working are emerging that promote a person-centred, collaborative approach, putting people in the driving seat in determining their own care needs. This contemporary module aims to facilitate the development of skills and knowledge, approaches and behaviour needed to support the multifarious needs of people living with chronic illness. It moves away from the traditional disease focused approach, to one that centres on the cross-cutting commonalities experienced by individuals living with long term conditions. The module is aimed at health and social care practitioners, and those with responsibility for care and management of adults with long term conditions.
