Faculty of Health Sciences Caring for People with Long Term Health Conditions Continuing Professional Development Individual module Level of study Please select a level How to apply What you'll study Health and social care practitioners need to be able to address the challenges of an aging population, providing care for patients/clients with long term conditions and the drive to provide more person-centred care outside of the hospital setting. This module will help to develop the knowledge base and skills needed to recognise and respond to the needs of individuals living with long term conditions.
